COVID-19 Screening Questionnaire and Considerations in Receiving Bodywork

According to reopening regulations, I need to ask the following questions for clients’ protection and mine. Keep in mind that your decisions regarding receiving appointments may affect the health of others I work with, many of whom are at high risk if exposed due to pre-existing conditions. If you have a high or moderate health risk according to Teton County Public Health Department guidelines, please also carefully weigh your benefits versus risks in attending appointments. Understand that pre-existing health conditions may increase your risk of infection and severity of illness should you become infected. I try to keep my exposure to others very low, and in full disclosure, I am now working as a volunteer at the Teton Raptor Center 1-2 times a week for a 3-hour shift.  We wear masks, and yet we are often standing within 6 feet of one another inside a trailer building. I have not tested positive for COVID nor been in contact with anyone who has in the last 30 days, and I will update you of any changes in my health status should it change within 14 days of our appointment.

You will be asked to answer these following questions as well as complete a waiver form when you come in for an appointment.

  1. Have you traveled outside Teton County and its contiguous areas in the last 14 days?
  2. Have you had any symptoms of viral illness, including any mild cold-like symptoms, in the last 14 days (fever, headache, chills, coughing, sore throat, sneezing, runny nose or sinus congestion, loss of taste/smell, weakness, fatigue, body aches, rapid/shallow breathing or difficulty breathing, sores on feet/toes, rash/hives, or hospitalization for stroke, seizure, heart failure or DVT/blood clot)?
  3. Have you been in contact with anyone with any of the above viral symptoms in the past 14 days?
  4. Do you have difficulty exercising to get your heart and respiratory rates up? 
  5. Have you had a new onset of muscle aches or pain since the emergence of the virus?
  6. Have you seen any new marks, rashes, spots, bumps, or other lesions on your skin?
  7. Have you been asked to quarantine by health officials in the last 14 days? 
  8. Have you tested positive for COVID-19 in the last 30 days?
  9. Have you been in contact with anyone who has tested positive for COVID-19 in the last 30 days? 
  10. Are you willing to come directly to your appointment from home having freshly showered and donned clean clothes? 

What to Expect When Coming for Your Appointment

In order to reduce exposure, we need to follow new guidelines suggested by the CDC and my professional organization, the Associated Bodywork and Massage Professionals. These will be our new prevention protocols to address new expectations in sanitization and decreased exposure for everyone concerned. I call our new protocol, “The Very Best  COVID Concierge Service.” (smile)

  1. Before your appointment, you will have answered the above questions in a response email and acknowledged that you have read this entire form and considered your decision to receive bodywork carefully and individually. Print, fill out, scan and email page one and page four of this form: I will file this paperwork in your client file, which is locked in a filing cabinet in my office.
  2. At the time of your appointment, we will conduct our friendly active listening and health pre-session intake over the phone while you’re in your car. We will try to keep these brief unless you desire a longer chat as part of your paid session.
  3. I will greet you at your car, provide you with hand sanitizer for you to conduct a 20-second hand rub, and have you don a new surgical mask. You will once again sanitize your hands.
  4. I will administer a forehead thermometer body temperature measurement and record this in my appointment notes as well as your repeated in-person (day of service) answers from the above screening questionnaire.
  5. In the suite and just outside my office, you will remove your shoes and use the sanitized spa sandals while walking in my office before and after lying on the treatment table.
  6. You will not sit in the waiting room nor use the hall communal bathroom but use my office’s private bathroom. Disposable cups will be provided for drinking water in my office’s private bathroom.
  7. You will wear the surgical mask for the entire length of your appointment and until you leave the building. No face or mouth work may be conducted at this time.
  8. There will be a nice wood box in which to store all your personal items, including coats, clothes, phones, jewelry, and etc.
  9. After undressing, wash your hands with soap and water according to the posted instructions, for 40-60 seconds, before getting on the treatment table. If you touch your face or facemask for any reason, please rewash your hands. Place any used tissues or paper towels in the bathroom waste basket with the foot-activated lid opener. This trash is removed after each client.
  10. If you use the toilet, please close the toilet lid before flushing and wash your hands with soap and water according to the posted instructions for 40-60 seconds.
  11. During the session, we will refrain from talking as much as possible. The room will be ventilated with a HEPA UV air filter, cracked windows in my office and bathroom, the bathroom fan, and a desktop fan. All linens will have been freshly laundered and delivered for your appointment, and pillows have cleanable covers. When face up, I will offer you the option of wearing sanitized plastic protective glasses.
  12. After your appointment and dressing, we will conduct a brief post-session interview in my office.
  13. Before exiting the suite, leave your sandals where you retrieve your shoes. Please sanitize your hands before you leave the building.
  14. You will receive an invoice to your email for the session which you can pay by credit or debit card. Payment is due on receipt and on the same day as service. Tips will be allowed. Most sessions will be 45 minutes to an hour. My rates are $100 for 45 minutes and $115 for 60 minutes.  I cannot offer a frequency or sliding scale discount for in-office visits for a while as they are very limited (unless you are using a package purchased prior to the COVID-19 shutdown). Remote appointments are offered on a sliding scale and also with the traditional package discounted rates as per my website.

My Sanitary and Safety Protocols

These are the procedures and tools I am using to meet new disinfecting requirements and in an attempt to provide you protection from exposure. Please understand that COVID-19 is a “nano-particle” airborne virus that remains suspended in the air for long periods of time (hence the need to ventilate as best I can in my office situation). Meanwhile, there are measures we can take to reduce surface to surface contact, including touching as little as possible in the office and not touching your face and washing hands often.

  1. My office air is filtered with a UV and HEPA filter air purifier. This will be on prior to, during and after appointments.
  2. All surfaces and soft materials are sanitized with a UV sterilizing wand.
  3. The carpet is vacuumed using a HEPA filter and an UV sterilizing attachment.
  4. Hard and vinyl contacted surfaces are cleaned and then disinfected with a proven industry germicidal and disinfecting cleaner.
  5. All sheets and coverings are laundered after each session. Any support pillows are double cased with a case that can be wiped down followed by a linen case that is laundered and replaced each use. Fresh linens are brought to the office the morning of your appointment.
  6. I use disposable gloves to remove and bag used linens and to clean all surfaces.
  7. My soft heating pad on the table is covered by a cleanable covering.
  8. I wear a KN95 mask followed by a fresh cotton washable mask and protective eye cover during your entire visit.
  9. I wear a fresh, clean smock for your appointment.
  10. I take my temperature prior to your appointment.
  11. I wash my hands before and after sessions and as I sanitize in stages.
  12. The room will be ventilated and air filtered following appointments.
  13. My oils and lotions are stored away and are only touched by myself. I sanitize any used bottles after each appointment.
  14. I keep up to date with health and safety information, monitor my health, and adhere to all closure orders.

Things that Will Remain the Same

  1. I will continue to support you in your healing as best I can and aim to serve with a clear mind, grounded energy, understanding, caring, information, intuition and ever-increasing knowledge and skills.
  2. I will offer aromatherapy in the room and individually. It’s still perceptible through the surgical mask.
  3. With much thought on the subject, I will maintain much of the ambiance of my treatment room. It will appear greatly the same yet pared down with all oils stored. My treatment room is neither a health clinic nor a doctor’s office, and its environment is to encourage relaxation, which is part of the healing process of bodywork.


There is an inherent risk in exposing one another via the air we breathe in close proximity to one another for a prolonged period of 45 minutes to an hour. I cannot promise your absolute safety. As an energy worker I can intend on causing no harm and ask for increased protection on behalf of myself and my clients. Physically, I can offer the reduced chance of harm through the above safety protocols, and I will hold these boundaries firm with all clients.  Washing hands often, wearing masks, and being aware of our health and others’ health will assist us in limiting exposure to COVID-19 and other viruses. Make the best decisions for yourself and also for others, as my exposure also means exposing my spouse and my other clients. I will also do my best to do the same.
